Job Description:
A company is looking for a Care Manager Transition of Care.
Key Responsibilities
Evaluate member needs through post-discharge assessments and
medication reconciliation
Develop care/service plans in collaboration with healthcare teams
to support member transitions
Facilitate communication and services between various healthcare
providers and resources
Required Qualifications
Master's degree in Behavioral Health or Social Work, or a degree
from an accredited School of Nursing
2-4 years of related experience in care management
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is
required
Preferred residence in Georgia, Tennessee, Nebraska, Oklahoma, or
Washington
Active RN license (compact license) is preferred
